Vendor Price Survey

The West Virgiina WIC Vendor Price Survey (VPS) is required of all applicant grocery vendors.  The purpose of the VPS is to ensure regional competitive pricing for all stores serving as WIC Authorized Vendors as required by the United States Department of Agriculture (USDA), Food and Nutrition Services (FNS).  This document is the official record of shelf prices. Vendors should check shelves while filling out this form.

Vendors should only list items which are on the shelf as of the date of completion for this VPS. For all categories of food, list the highest shelf price of the item which is West Virginia WIC approved, except for milk. For milk, list the lowest price of milk in the fat content/flavor group. For categories of food which specify brand names, list the shelf price of that brand.  Please include the product name, package size, 12-digit Universal Product Code (UPC), and the price.

You must report products and prices for items necessary to meet WIC Minimum Stock Requirements.  Leave blank any food category or items which your store does not have in stock.
